Women who should take "DUTCH Complete"

1. Surgical Menopause - Low hormone levels and symptoms of menopause caused by ovary removal or total hysterectomy.

2. Peri-Menopause – The 2 to 7 years just prior to menopause often consisting of irregular menstrual cycles and uncomfortable symptoms due to fluctuating hormone levels (increased levels of estrogen relative to progesterone).

DUTCH vs. Saliva Testing

While the free cortisol pattern in saliva has clinical value, there is a significant missing piece to surveying a patient’s HPA-Axis function with saliva testing – measuring cortisol metabolites. To properly characterize a patient’s cortisol status, free and metabolized cortisol should be measured to avoid misleading results when cortisol clearance is abnormally high or low. Likewise with sex hormones, measuring estrogen and androgen metabolites gives a fuller picture for more precise clinical diagnosis of hormonal imbalances and HRT monitoring.

DUTCH vs. Serum Testing

While the most universally accepted testing method (due to the availability of FDA-cleared analyzers that are reliable and inexpensive), serum testing is lacking in some areas. Adrenal hormones cannot be effectively tested in serum because free cortisol cannot be tested throughout the day. There is also a lack of extensive metabolite testing (especially for cortisol and estrogens).

DUTCH vs. 24-Hour Urine Testing

There are two primary drawbacks to 24-hour urine testing of hormones. First, the collection is cumbersome, and as many as 40% of those who collect, do so in error (Tanaka, 2002). Secondly, dysfunction in the diurnal pattern of cortisol cannot be ascertained from a 24-hour collection. Some providers add saliva for daily free cortisol. DUTCH eliminates the need for two tests.

Estradiol, Estriol & Estrone - Made by Men & Women

The Hormone that Impacts Reproductive Organs

Estradiol (E2), Estriol (E3), and Estrone (E1) are types of bio-identical estrogen. Both men and women make estrogen. High levels of estrogen in men can disturb prostate function. 

Estriol is the weakest of the three estrogens and is shown to have protectant qualities and help with menopausal symptoms. Estriol or estradiol is sometimes used vaginally for vaginal dryness. Estradiol is the primary estrogen involved in the relief of menopausal symptoms like hot flashes, decreased energy, depression, osteoporosis and heart disease. Estrone is a potent estrogen that increases after menopause.

Cortisol & Cortisone

A Hormone Tied to Mood and Weight Gain

Typically, as one gets older, their waistline changes and this is because fat cells in the abdominal area of your body are extremely sensitive to cortisol fluctuations. High cortisol leads to fat accumulation in the abdomen. Cortisol is active, cortisone is inactive and DUTCH evaluates both and finds out which hormone is predominant. A waking urine sample allows represents the entire night! The next sample is taken 2 hours later and you should see a surge in active cortisol. A total of 4 samples allows DUTCH to give you your 24 hour cycle with peaks and troughs. Evaluating the actual metabolites of cortisol can give you an idea (free cortisol being low, and metabolized cortisol is relatively high, this means there is a lot of cortisol clearance which is common in obesity, hyperthyroidism and more). 

There is also a strong correlation between dopamine and stress. During chronic stress, your body is initially flooded with cortisol, resulting in a dopamine depletion. It turns out that your body will basically replace dopamine with the excess cortisol. This results in anxiety, depression, memory loss, heart disease, higher risk of auto-immune conditions and low pleasure. Pills don't fix this, evaluating cortisol could.

Progesterone & Pregnanediol

Helps You Sleep and Reduce Inflammation

Progesterone is well known for its ability to protect women's breast and uterine health. Men should also be evaluating progesterone though. It's needed for bone mass, joint comfort, and relaxation. Progesterone is helpful for insomnia because it activates GABA receptors. GABA is one our most calming neurotransmitters. DUTCH evaluates levels of progesterone! 

Progesterone is a very strong anti-inflammatory and can suppress IL-4 and IL-10 as well as the Th1 cytokine response. It's a neuroprotective hormone. Progesterone is rarely evaluated in many clinical practices. It needs to be acknowledged and kept in proper balance with estrogen (which should NEVER be given unopposed). Estrogen should always be given with progesterone because all human cells have receptors for progesterone. Pregnanediol is also evaluated by DUTCH and this is an inactive metabolite of progesterone. This gives you another indirect evaluation of progesterone levels in your body. It's a perk to measure both these biomarkers.

Testosterone - Made by Men & Women

A hormone very important for sex drive, fertility, stamina and bigger muscles. It's usually associated with men, however, women need testosterone too! It can improve mental clarity, bone mass and increased energy and endurance.

Testosterone levels can decrease by 50% in post-menopausal women or young women who have undergone a hysterectomy. Declining levels of T can cause a sudden lack of sex drive, mental fatigue and bone loss. Menopausal symptoms like night sweats, insomnia, and vaginal dryness can also affect libido. 

Glutathione Marker called Pyroglutamate

 The DUTCH Complete test evaluates your glutathione by measuring your pyroglutamate levels. Either very high levels of urine pyroglutamate OR extremely low levels may indicate glutathione deficiency. Glutathione is a liver enzyme that helps your body naturally reduce oxidative (free radical) damage. It can improve levels of energy and serve to reduce achy muscles and inflammation. Glutathione is a poison-neutralizing enzyme, and a potent antioxidant that, according to the Journal of Cancer Science and Therapy, is useful in reducing risk of cancer. Elevated glutathione levels raised antioxidant levels and thus, resistance to oxidative stress in cancer cells. Evaluating your status is useful.

A hormone produced by the adrenal glands. It is involved in immune function, memory, libido, and prevention of heart disease and bone loss. 

DUTCH is unique because it gives you a very thorough result by measuring 3 different biomarkers to give you a full picture of your DHEA levels. (It's not a snapshot based upon your blood, which changes every few minutes). DHEA makes 3 metabolites and DUTCH measures them: Androstenedione, DHEA-S and etiocholanolone and together these add up to a truer measure of your "Fountain of Youth" hormone. The levels should be different depending on your age, and DUTCH tells you this. Remarkable!

Dopamine, Epinephrine & Norepinephrine 

Neurotransmitter Metabolites for Mood and Energy

Dopamine status as well as other catecholamines cannot be measured accurately by the blood so evaluating urine metabolites is a better way to evaluate these neurotransmitters which are involved in many conditions including depression, Alzheimer's and Parkinson's.

* Dopamine - DUTCH testing measures a urine biomaker called "homovanillate" or HVA. Low levels indicate lower dopamine function because either the dopamine is breaking down too fast, or there isn't a whole lot present.
* Norepinephrine/epinephrine - Urine biomaker called VMA (Vanilmandelate) will give you a picture of these neurotransmitters. Epinephrine is another name for adrenalin.
* Serotonin - 5HIAA - Hydroxyindoleacetic acid is used to indicate levels of serotonin. This is a neurotransmitter associated with self esteem, pain and happiness.

B6 Biomarker - Xanthurenic acid will rise if you are deficient in vitamin B6.

B12 Biomarker - Methylmalonic acid (MMA) is your body's biomarker for B12 and will rise if you are deficient in vitamin B12. 

5 Alpha Path - Are You Going Down this Pathway?

When testosterone (a potent androgen) goes down a pathway called "5 alpha, a man or woman will become more androgenic. In the case of testosterone going down the 5 alpha pathway... it creates more DHT (dihydrotestosterone) and this makes a person more androgenic!

In men, preferential breakdown via the 5 alpha pathway can lead to prostatitis and symptoms of BPH (benign prostatic hypertrophy), or other prostate problems.

In women, this will lead to facial hair, acne, thinning scalp hair. Having this information allows your practitioner to find out how much DHEA, testosterone your making and is there a preference for "5 alpha" metabolism. DUTCH will tell you this.

You should remain on your hormones while testing, do not stop them. If you remain on them, the DUTCH test will show you how you’re doing on the hormones that you take. Some people think they need to stop their hormones and get off everything, but if you do that, the DUTCH report tells you how you’re doing off of them, and that is not really useful. You want to know how you’re doing with what you’re taking, and that way you’re practitioner knows whether to raise the dose or lower it.

Let's look at real cases now...

This is a post-menopausal woman. Her values should fall somewhere in the purple section. For example, she should have Estradiol between 0.2 - 0.7 on the wheel. But she is below that. She is also below the purple section on progesterone. And further, She is below range on Testosterone. Her ideal range would be somewhere between 2.3 and 14 on Testosterone. It can be surmised she is not on any Hormone Replacement Therapy (HRT) or she is taking something but it is not responding. Symptoms would include hair loss, faster aging, bone loss, fatigue, insomnia and possibly joint pain and poor memory recall. Natural boosters for estrogen (for example maca or flax) may be useful. Progesterone boosters or medications may be of help too. Furthermore, it is useful to have a physician weigh in to determine if HRT is necessary based upon individual symptomatology.

Managing hormones is important, especially if you are experiencing symptoms that are chronic. Be aware that taking estrogen-containing hormones can also increase the risk for gallstones/gallbladder disease, cysts, fibroids and reproductive inflammation. So keeping an eye on your hormones and the way that they break down is important... especially because they can be managed sometimes with natural herbs/vitamins and antioxidants.

3 States Have Special Requirements:

⚠️ If you live in the state of Maryland 🦀 

In Maryland, there are ONLY two ways you can get the DUTCH Complete test:

1. Order the test from HERE, and get a prescription (Rx) from a Maryland medical doctor for the DUTCH Complete lab test then send the prescription back when you send in your samples (ie your completed test kit). Just include the Rx. The Rx is required for Maryland residents only.

2. If your Dr. will not write you a prescription for this test, or you don't want to work with a physician there, order it directly from a Maryland practitioner. Here's a LINK where you can find providers in the state of Maryland.

⚠️ If you live in the state of New York 🍎

In the state of New York, you are required to sign an affidavit and send it along with your DUTCH dried samples when you send them to the lab. This is because New York has restrictions on their residents such that they cannot do certain at-home lab test kits. So the affidavit states that you verify that you have driven or flown some place else (out of state) and collected your urine samples outside the state of New York! You must print out THIS AFFIDAVIT and sign it, and send it in the pre-paid envelope back to the DUTCH lab with your dried urine samples. That way DUTCH has what they need in order to process your specimen, and email you the results like normal. If you live in New York, you do NOT need a prescription like Maryland residents need. 

⚠️ If you live in the state of Rhode Island ⚓

In the state of Rhode Island, the rules are similar to New York. So you are required to sign an affidavit and send it along with your DUTCH dried samples when you send them to the lab. This is because Rhode Island has restrictions on their residents such that they cannot do certain at-home lab test kits. So the affidavit is you stating that you have definitely urinated out of state, (that is to say, OUT of Rhode Island and not in New York either). You must print out THIS AFFIDAVIT and sign it, and send it in the pre-paid envelope back to the DUTCH lab with your dried urine samples so that DUTCH can process your specimens, and email the results to you.

If you live in Rhode Island, you do NOT need a prescription like Maryland residents need.

In the country of Canada, you have to use a special shipping label which you can retrieve by CLICKING HERE. When you click on that link, you will see that you have two choices:

1. Print a return label to affix to your package
2. Generate a custom QR code for UPS to print the label for you.

These are pre-paid UPS labels that will return your completed urine strips (ie your samples) to Precision Analytical laboratories. You will need to use UPS as your shipping method and there is a link from the same page to help you find UPS locations near you.
